Alcoholism is a serious and fatal medical condition. It greatly diminishes the quality of life as it affects health, work, and family. Choosing to enter an alcohol rehabilitation center and seek professional treatment can help sustain the decision to recovery. Admitting that one is dependent and recognizing the need to stop and abstain is a great first step. But an alcoholic will need the help and support of his loved ones, family, and friends.

There are public, private, and non-profit treatment centers that differ from one another in regards to their philosophy, qualifications, and programs. Selecting which alcohol rehabilitation center is a crucial first step as this would determine which direction, and most importantly, the outcome of the treatment. One thing to know about these centers is that there are no locked doors and every individual is free to leave during the course of rehab if they choose. No treatment of any kind will work on an individual if he doesn't want to be treated. Voluntary willingness is a key to treating alcoholism.

No one exactly knows how to differentiate one center from the other. It is important to consider some factors in researching the appropriate treatment centers and programs. Apart from the cost, locations, and length of treatment, choose a center that is recognized and accredited. It can also be comforting to research on their success rates. The center's philosophy on rehabilitation greatly influences their operations and is usually a tell-tale sign about their programs.

Every alcohol rehabilitation center espouse that recovery is a life-long process and there are no magic bullets. There are usually four general phases in a rehab program. First is the commencement of treatment. Denial, rationalization of alcohol dependency, and hesitation to give up this dependency characterizes the first stage. The second step is initiating abstinence from alcohol. Individuals will have to confront cravings, withdrawal symptoms, and other psychological factors and fight off temptations resulting to a relapse. Counselors and therapists claim this to be the hardest of all stages. Maintaining abstinence is the third step. It is usually a series of counseling and learning the signs of a relapse. The fourth phase is the advanced recovery stage where individuals are now able to remain sober and have learned the skills to become better persons, parents, and friends.

A randomized controlled trial was conducted to determine whether a manualbased
psychodynamic treatment, labeled dynamic deconstructive psychotherapy
(DDP), would be feasible and effective for individuals with co-occurring borderline
personality disorder (BPD) and alcohol use disorder. Thirty participants
were assessed every 3 months during a year of treatment with either
DDP or treatment as usual (TAU) in the community. DDP participants
showed statistically significant improvement in parasuicide behavior, alcohol
misuse, institutional care, depression, dissociation, and core symptoms of
BPD, and treatment retention was 67% to 73%. Although TAU participants
received higher average treatment intensity, they showed only limited
change during the same period. The results support the feasibility, tolerability,
and efficacy of DDP for the cooccurring subgroup and highlight the
need for further research.

A ferry has capsized during a freak storm in the central Philippines, leaving 39 people dead, police say.

The inter-island ferry carrying more than 100 commuters overturned in strong winds off the island of Masbate, south-east of the capital, Manila.

Officials say 76 people have been rescued, and a search is under way between Masbate and Luzon islands.

The ferry had been sailing for about 20 minutes in clear weather when a sudden gust caused it to keel over.

"The Don Dexter Kathleen capsized due to a freak accident," AFP news agency quoted senior police superintendent Ruben Sindac as saying. "It was hit by a high wind despite fair weather and calm waters."

The ferry had 119 passengers and crew on its manifest, but Supt Sindac said he would not rule out the possibility that more people were on board.

The bodies of the victims were taken to Dimasalang town, about 5 km (three miles) from the accident site, where wailing people began looking for missing loved ones.

Maritime accidents are common in the Philippine archipelago due to tropical weather, badly maintained passenger boats and weak enforcement of safety regulations.

Hundreds of people were killed in June, when a 23,000 tonne ferry - Princess of the Stars - capsized during a typhoon off the central island of Sibuyan.

Only 57 passengers and crew of the 850 people aboard survived the accident, making it the worst maritime disaster in the Philippines for 20 years.

Dr. Earl Bradley, 56, who has had a practice in Lewes, Delaware, for more than 10 years, is charged with eight counts of first-degree rape; four counts of second-degree rape; 14 counts of sexual exploitation of a child; and two counts of endangering the welfare of a child, Delaware Attorney General Beau Biden told CNN.

A prosecutor in Biden's office estimated that Bradley may have had as many as 100 victims. Biden stopped short of that, saying only authorities think there may have been multiple victims, and the investigation is ongoing.

As of Wednesday, seven victims had been identified, said Jason Miller, spokesman for the Delaware Department of Justice. CNN affiliate WBOC-TV reported Bradley is being held in lieu of $2.9 million bond.

He was scheduled for a preliminary hearing Wednesday in the Sussex County Court of Common Pleas, but his attorney requested that it be continued until January 14, Biden said. WBOC reported that concerns about Bradley's mental health prompted the postponement and said the doctor is under suicide watch at Sussex Correctional Institution.

The charges against Bradley, and any additional charges that are filed, will be presented to a grand jury "at some point," Biden told CNN.

He would not give specifics of how Bradley came to authorities' attention "over the course of recent months." Police believe the abuse goes back "for several years," with victims as young as 2, he said.

Additional charges were filed earlier this week after "preliminary forensic investigation of a computer" seized by Delaware State Police, authorities said in a release.

Police said they have seized photographic and video evidence showing Bradley abusing victims, WBOC said. Court documents provide graphic details about the evidence, the station said.

Bradley also has medical licenses in Pennsylvania, New Jersey and Florida. Biden said authorities in those states have been contacted.

Biden urged parents or others who have concerns regarding Bradley to contact his office, where a hot line is staffed with people who can provide information including counseling referrals.

"We're taking this incredibly seriously," he said.

PESHAWAR, Pakistan (AFP) – The Taliban blew up a girls' school in Pakistan's Khyber district, where troops are fighting against militants in the tribal region bordering Afghanistan, an official said Wednesday.

Militants detonated explosives overnight at the government-run school in Bazgarah town, about 40 kilometres (25 miles) west of Peshawar, capital of the violence-plagued North West Frontier Province.

"The building had 21 rooms. All have been completely demolished," local administration chief Shafeerullah Wazir told AFP by telephone.

There were no casualties because the property was empty at the time.

"Taliban and their local allies are responsible. They are destroying educational institutions to avenge the military operation against their hideouts in the area," said Wazir.

"This was the ninth educational institution blown up in Khyber over the past six weeks," he added.

Islamist militants opposed to co-education and subscribers to sharia law have destroyed hundreds of schools, mostly for girls, in northwest Pakistan in recent years.

The fabled Khyber tribal region is the main land bridge to neighbouring Afghanistan and the principle supply route for NATO troops fighting an eight-year Taliban insurgency across the border.

Pakistani troops launched an offensive in Khyber in September in a bid to flush out the Taliban and homegrown militant group Lashkar-e-Islam (Army of Islam) led by local warlord Mangal Bagh.

The United States is increasing pressure on Pakistan to crack down more on militants hunkered down in its lawless tribal belt, branded the most dangerous place on Earth and the chief sanctuary of Al-Qaeda.

LOS ANGELES – One of Hollywood's most enduring couples has separated.

Susan Sarandon and Tim Robbins, partners for 23 years and parents of two sons, split up over the summer, publicist Teal Cannady said in a statement Wednesday. She did not elaborate.

Sarandon, 63, and Robbins, 51, met while shooting the 1988 film "Bull Durham." He played a hotshot pitcher, she was the passionate fan who simultaneously seduced him and prepared him for the big leagues.

Sarandon and Robbins never married. Instead, they have been compared to other longtime Hollywood pairs who remain committed despite never officially tying the knot, such as Goldie Hawn and Kurt Russell.

Brittany Murphy spent years denying eating disorder allegations but even she worried about her scarily low weight in the weeks leading up to her death.

"I am a bit thinner now than what I would like to be," Murphy, 32, admitted to Fox News' Pop Tarts at an event December 3. Then: "I was a ballerina for a long time. I still take ballet lessons now - what it does to your body is incredible."

Murphy's big break came in 1995 when she starred in 'Clueless' as Tai and looked, refreshingly, like someone who didn't fit the Hollywood mold into which she'd later shove herself. As her career took off her weight plummeted while her chest was bigger than ever, she dyed her hair blonde, capped her teeth and appeared to have had plastic surgery although she repeatedly denied having been under the knife. As she brushed off persistent rumors that her ever-shrinking form was the result of an eating disorder.

"I'm the same size I was in Clueless; it's just that the weight in your face changes as you grow and get older," she said. "This is my body. I'm proud of it ... I'm healthy."

The photos below show that Murphy became thinner, dramatically so, as she tried to find her place in Hollywood.

Clueless' director Amy Heckerling told blogger Scott Feinberg:

"Maybe she felt like she was not the, like, skinny, pretty girl, you know? And then the next few movies she was, you know, thinner, blonde, you know, at MTV Awards and all sorts of things like that, and going out with Eminem and Ashton Kutcher, and, you know, suddenly got more into that whole glamorous scene... I think she felt the pressure to become a different sort of commodity to survive in show business, and I think it was awful."

Murphy's mother told paramedics that her daughter suffered from diabetes. If true, this could have played into any body image problems as many young female diabetics use the dangerous practice of skipping insulin injections to control their weight.

While Murphy repeatedly denied the drug allegations that plagued her career, those around her paint a different picture. "She had too many drugs and too little food," one of her makeup artists told the Daily Beast.

Murphy was looking more frail than ever at her last public appearance. Nutritional deprivation, especially coupled with vomiting, which Murphy was experiencing at the time of her death, can result in potassium levels low enough to cause cardiac arrest.

An autopsy will soon determine which of these factors contributed to Murphy's fatal heart attack.

The investigation explored whether subtypes of relapse to violence exist for different levels
of intimate partner violence and drinking behavior among men who relapse to both alcohol
and violence after alcoholism treatment. Male clients entering an alcoholism treatment
program who reported at least one incident of intimate partner violence and their female
partners ( N = 294) were recruited for participation. Data were analyzed using a latent class
analysis of mixture model. Findings revealed that two classes of violence best described
the sample of men that relapsed to violence. Class 1 contained males who had perpetrated
more days of violence, relapsed to violence faster, more frequently relapsed to alcohol,
and had a higher percentage of males identified as meeting diagnostic criteria for antisocial
personality disorder compared to those in class 2.

Objectives: To examine the reliability and validity of a new measure of alcohol outcome expectations for college students, the Alcohol Consequences Expectations Scale (ACES). Methods: College students (N=169) completed the ACES and several other measures.
Results: Results support
the existence of 5 internally consistent subscales. Additionally,the ACES is associated with conceptually similar measures and self-reported drinking behavior. Conclusions: This study supports the reliability of the ACES and its subscales and provides preliminary evidence of construct and criterion-related validity. Pending further investigation, this scale may be used to inform the development of alcohol abuse prevention programs on college campuses.

BAGHDAD (Reuters) – A U.S. commander in northern Iraq does not expect to order a court martial for soldiers who become pregnant, but has not rolled back a controversial new policy on pregnancy, a military spokesman said on Wednesday.

A new directive from Major General Tony Cucolo, who commands U.S. soldiers in northern Iraq, sets out possible punishments from reprimand to court martial for prohibited behavior, including drinking alcohol, taking drugs or becoming pregnant.

The policy has been criticized by some women's advocates and on Tuesday four U.S. senators wrote to the secretary of the U.S. Army on Tuesday asking that it be rescinded.

"We can think of no greater deterrent to women contemplating a military career than the image of a pregnant woman being severely punished simply for conceiving a child," Senator Barbara Boxer and others wrote.

Cucolo defended the rules, which took effect for his 22,000 soldiers when he took over in northern Iraq in November, as necessary to retain combat power as U.S. forces prepare to withdraw from Iraq by the end of 2011.

Cucolo's command includes some of the most dangerous areas of Iraq, where ethnic and sectarian rivalries have fueled an ongoing insurgency.

Military spokesman Major Jeff Allen said Cucolo, who personally decides what punishment soldiers violating rules will receive, had clarified that he did not intend to court martial any of his soldiers who became pregnant.

"I have not ever considered court-martial for this. I do not ever see myself putting a soldier in jail for this," Cucolo said in a conference call on Tuesday with reporters.

Yet Allen said that Cucolo, contrary to what some media reports had implied, had not changed his stance. Becoming pregnant remains on the list of prohibited behavior that could result in a variety of punishments including criminal charges.

Four of Cucolo's soldiers who were found to be pregnant since the November 4 order took effect had received letters of reprimand, Allen said. Three male soldiers involved were also reprimanded, and one of them received a more serious reprimand because he had committed adultery.

"The fact that the soldiers received letters of reprimand does not represent a change in policy, and no changes have been made to the directive," he said.

The policy applies only to soldiers under Cucolo's command. U.S. Central Command rules do not prohibit "sexual contact between consenting, single servicemembers," but individual commanders can issue rules that are stricter than that.

"That is what Major General Cucolo had done in this case; he has taken a policy and made it stricter to meet his desire to send a message to our soldiers on this issue," Allen said.

Abstract:
Cigarette smoking is widely prevalent among individuals
in treatment for drug or alcohol dependence; however, the treatment
of nicotine addiction in this population has numerous obstacles at
both programmatic and patient levels. Despite these difﬁculties,
recent studies have demonstrated moderate success in implementing
smoking cessation treatment in drug rehabilitation programs. The
National Drug Abuse Treatment Clinical Trials Network sponsored
a smoking cessation study in 13 community-based outpatient sub-
stance abuse rehabilitation programs across the country. The study
evaluated the effectiveness of smoking cessation treatment provided
as an adjunct to substance abuse treatment-as-usual. This report
summarizes the practical and clinical experiences encountered at
each of the study sites with regard to implementing the smoking
cessation treatment intervention. Smoking behavior of the treatment
clientele was assessed by anonymous survey at each site. In addi-
tion, sites were systematically characterized by using program re-
view and assessment tools completed by the respective staff and
program directors at the site. Survey and recruitment data indicated treatment is more feasible, in methadone maintenance treatment
programs. Other factors associated with smoking behavior and with
the recruitment of drug- and alcohol-dependent individuals into the
smoking cessation treatment study are described.

Background: The safety of small amounts of alcohol drinking and
occasional binge-level drinking during pregnancy remains unsettled.
We examined the association of maternal average alcohol intake and
binge drinking (􏰀5 drinks per sitting) with infant mortality, both in
the neonatal and postneonatal period.

Methods: Participants were 79,216 mothers who were enrolled in
the Danish National Birth Cohort in 1996 –2002, gave birth to a
live-born singleton, and provided information while they were
pregnant on alcohol consumption during pregnancy. Information on
infant mortality and causes of death was obtained from national
registries and medical records.A

Results: During the ﬁrst year of life, 279 children (0.35%) died, 204
during the neonatal period. Infant mortality was not associated with
alcohol drinking, even at a consumption level of either 4+ drinks
per week or 3+ occasions of binge drinking. Postneonatal mortality
was associated with an intake of 4+ drinks per week (hazard ratio
=3.56 [95% conﬁdence interval + 1.15– 8.43]) and with 3+ binge
episodes (2.69 [1.27–5.69]). When restricting analyses to term
births, both infant mortality and postneonatal mortality were asso-
ciated with a weekly average intake of 4+ drinks or 3+ binge
episodes.

Conclusions: Among term infants, intake of at least 4 drinks of
alcohol per week or binging on 3 or more occasions during preg-
nancy are associated with an increased risk of infant mortality,
especially during the postneonatal period.

Do multivitamin supplements modify the relationship between prenatal alcohol intake and miscarriage?

OBJECTIVE: To determine whether multivitamin supplements modify
the relationship between alcohol consumption during pregnancy and
the risk of miscarriage.
STUDY DESIGN: We used data from a population-based cohort study of
pregnant women (n �� 1061; response rate �� 39%). Participants were
asked about their alcohol consumption and vitamin intake during
pregnancy.
RESULTS: Among multivitamin nonusers, women who drank alcohol
during their pregnancy were more likely to have a miscarriage com-
pared with women who abstained (adjusted hazard ratio, 1.67; 95%
conﬁdence interval, 1.04 –2.69). However, among multivitamin users,
there was no difference in the risk of miscarriage between alcohol con-
sumers and abstainers. Results suggest the volume of alcohol as well
as the timing of multivitamin supplementation may also be important.
CONCLUSION: Our ﬁndings suggest that a woman of childbearing years
might decrease her risk of miscarriage associated with alcohol intake by
taking multivitamin supplements. However, our ﬁndings should be in-
terpreted with caution and future research replicating these ﬁndings is
necessary.
